The primary issues are usually an incomplete application or website doesn't match with the product. You will find typically even if you application wasn't accepted the first time, send an email explaining why you feel you are a good fit for their program, you will find that most will get back to you with a yes. The other thing to note is the affiliate manager is attempting to weed out the fraudulent affiliate applications so everything you can do at your end to help them understand you're for real the better your chances are of being accepted.
